NURS FPX 6218 Assessment 4 Advocating for Lasting Change
Identification of Stakeholders
Identifying stakeholders is crucial as they are directly involved in the change management process (Suhaib, 2019). The stakeholders are identified based on their influence, power, and urgency related to the change proposal. They help arrange funds and resources for implementing the proposed change in the community (Gregory et al., 2019). The stakeholders for the change proposal are:

Policy and Financial Support
Jordan is situated in Scott county Minnesota, whose population is 7,013 in 2023 (World Population Review, n.d.). The people in the community are facing issues in getting medical care services due to poverty (14.2%) and lack of health insurance (U.S. Census Bureau, 2021). The lack of medical care services will result in health disparities, infections, and mental health problems (Capella University, n.d.). The community needs professionals and experts like dermatologists, skin specialists, and dentists (Data USA, n.d.). The store and forward technology will make accessing medical services easier for people to access health care in Jordan’s community center. Implementing the change proposal is possible due to financial support from health agencies, corporate donations, and grants. Support from Vila Health agencies, corporate donations, Affordable Care Act (ACA), Medicare, and Medicaid policies will be needed to implement the proposed change in the community (Limbach & Fong, 2022). The policies and governmental organizations provide donations, funds, resources, expansion programs, and insurance plans. The plans help low-economic groups to get medical care services (Tilhou et al., 2020).
Policy Change and Fundings Needed
Funding and policy change is needed as the Jordanian community lacks resources, funds, and specialized healthcare experts to provide medical services to patients (Capella University, n.d.). The need for external help from organizations and associations is required to implement technology successfully. The proposed change will help the people in the Jordan community to get healthcare services from experts. The funding is also needed because of the poverty issues (14.2%) in the Jordan community, which hinders people from getting medical care (U.S. Census Bureau, 2021). The store and forward technology assist people in getting affordable and cost-effective healthcare services (López Seguí et al., 2020).
Challenges or Concerns
Some challenges or concerns may encounter in implementing the proposed change in the community. One of the challenges is the need for more awareness and knowledge among community members. It causes resistance to accepting digital technology (Talwar et al., 2020). Some privacy and security concerns are related to the technology, including data breaches and losing confidential information to unauthorized personnel (Keshta & Odeh, 2020). Another issue is the need for more technical assistance among healthcare staff regarding the use of technology. The patient’s condition may deteriorate due to the non-provision of healthcare. The disturbance in health conditions increases hospital readmissions and financial burden. Addressing barriers and concerns is crucial to implement the technology. The barriers can be addressed by implementing the store and forward technology. Technology will help provide patient care services (Wang et al., 2020).
Assumptions that Support the Rationale for Policy Support
The underlying assumption is based on the results from the Windshield survey. The survey results showed the need for improvements in the community in getting medical care services (Capella University, n.d.). Another assumption is that the lack of healthcare services results in health disparities among a low-socioeconomic group. The assumption supporting the need for policy support is providing healthcare services to all community members to reduce the chances of infections and associated diseases. Healthcare services should be provided to all people without any social, economic, racial, or cultural differences, reducing the financial burden and health disparities (Snoswell et al., 2020).
Social Determinants of Health
The SDOH (Social determinants of health) is the factors that influence individuals’ health and lifestyle. The SDOH consists of resources for providing education and healthcare services to improve health outcomes and quality of care (Burström & Tao, 2020). The Windshield survey evaluated the SDOH in the Jordan community. The SDOH in the Jordan community is the low-socio-economic status of people, discrimination due to differences in social, racial, or cultural differences, and lack of medical services to patients (Capella University, n.d.). Implementing the store and forward technology will help address the SDOH in the community by providing medical care services (Koch et al., 2022).
Windshield Survey and Environmental Analysis Findings
The survey findings and environmental analysis identify the needs and resources for improving outcomes and quality of care services (Capella University, n.d.). The Windshield survey elaborated that Jordan has schools, residential areas, city councils, parks, and a center for assisted seniors. These above are the positive aspects of the community. Some factors need improvements, and some opportunities are available for establishing the business. According to the survey reports, some negative aspects are the low-socio-economic status of people, discrimination due to social, racial, or cultural differences, and lack of medical services for patients (Data USA, n.d.).
Environmental Analysis Findings
The community’s environmental analysis findings explained that air and water quality needs improvements (Capella University, n.d.). The water is contaminated as it contains metals, metal oxides, and other contaminants (Data USA, n.d.).Due to contamination, the condition of patients is deteriorating and increases the risk of diseases like cholera, hepatitis, and skin infections. There is a chance of health disparities due to the non-provision of healthcare services (Wang et al., 2020). The use of a store and forward technology will help people access medical services in Jordan’s community centers. The analysis and findings help encourage the patients and community members about the store and forward technology. The community’s vacant area helps establish business and economic stability (Capella University, n.d.).
The health concerns and issues, including the low-socio-economic status of people, discrimination due to differences in social, racial, or cultural differences, and lack of medical services to patients, are needed to address in the Jordan community (Capella University, n.d.). There is a need that existing policies must address the change proposal of the store and forward technology in the Jordan community to improve health outcomes and quality of care. The technology will help primary care providers get expert and professional assistance. It will help the low-socioeconomic group to get healthcare services without cultural, racial, or social differences. As a result, the proposed change help to improve outcomes and access to medical care services (Wang et al., 2020).
NURS FPX 6218 Assessment 4 Advocating for Lasting Change
The literature review shows that various studies encourage and support the effectiveness of store and forward technology in improving health outcomes and standard of care. A study by Eze et al. (2020) demonstrated the role of the store and forward telemedicine technology in reducing costs and improving patient experiences with the treatment approaches. The study demonstrated that policymakers expect tele-health technology to improve healthcare services (Eze et al., 2020).
Benefits of the Change for Community and Stakeholders
The proposed change positively influences the community of Jordan and relevant stakeholders. The technology will provide care services to all people without discrimination. The low socioeconomic group of people gets medical services that are affordable and cost-effective. The travel cost and financial burden will be reduced. Nurses’ reduced workload and burnout also reduce the financial burden due to travel and expensive treatments (Gillespie et al., 2019).
Support from Vila Health agencies, corporate donations, Affordable Care Act (ACA), Medicare, and Medicaid policies is needed to implement the community’s proposed change. The financial support and resources assist in sustaining the proposed change of store and forward technology (Limbach & Fong, 2022). The training workshops and seminars are conducted to improve awareness and knowledge among healthcare staff and patients regarding the store and forward technology. The budget plan is crucial in analyzing the human resource, incentives, and consultation costs for implementing the technology. Implementing the technology requires financial support (Siyal et al., 2019).
The estimated cost for implementing the store and forward telehealth technology varies depending on the needs and available resources and may be more than $ 150,000 (Ravitz et al., 2021). The anticipated budget plan costs for the start-up and the first year are $262,500 and $233,000, respectively, as was previously determined in the assessment. The budget plan covers the salary and wages of project managers, healthcare experts, nurses (assistants and informaticians), and nurse managers, which are $45,000, $85,000, $25,000, and $30,000, respectively. The fringe benefits include financial incentives and medical coverage, which is $15,000. The consultation and software installation costs $4,000 and $18,000, respectively. The suppliers and training workshops include $10,000 and $1000, respectively.
Plan for Leading Transformation Change
The proposed change plans aim to provide medical care services to all members of the Jordan community. The plan will reduce the financial burden and traveling costs due to the treatment approaches on patients and their families. People of low socioeconomic status and vulnerable populations in the community get affordable medical care services (Gillespie et al., 2019).
Evidence-Based Transformational Change
In the change proposal implementation, evidence-based transformational change is carried out through the ADKAR change model (Wong et al., 2019). The model will assist in reducing the barriers and concerns regarding the change proposal. It helps in the creation of transformational change in the Jordan community. It has five steps: awareness, desire, knowledge, ability, and reinforcement (Wong et al., 2019).
1.Firstly, there is a need to create awareness among the members regarding the store and forward technology.
2.Secondly, there is a need to create a desire that the change proposal helps improve outcomes.
3.Thirdly, there is a need to increase the knowledge about the role of technology.
4.In the fourth step, the members must create a sense of ability to implement technology.
5.In the fifth step, the implementation of technology is reinforced in the community (Wong et al., 2019).
NURS FPX 6218 Assessment 4 Advocating for Lasting Change
The Jordan Community Center is being modified with the help of technology. The adoption of new technology modifies the organizational culture and idea of using community centers for recreational purposes. Healthcare services will be provided to all the community members. The role of transformational change assists in creating a healthy environment to enhance outcomes (Robbins & Davidhizar, 2020).
Ensuring Ongoing and Effective Communication
The use of verbal, non-verbal, and open communication help in ongoing effective communication among community members and associated stakeholders (Ahmed, 2020). The important people that help implement technology are project managers, groups of nurses (assistants, informaticists, and nurse managers), healthcare experts, and corporate funding agencies and grants from organizations and associations. Following the change management model by all the members is necessary to implement technology successfully. A study by Hubbart (2022) evaluated the importance of the ADKAR change management model in changing the organizational culture and arranging buy-in from the funding agencies.
Community Health Complications
The purpose of Healthy People 2030 is to provide cost-effective health services for all individuals by 2030 (Healthy People 2030, 2020). It emphasizes the reduction of health disparities through leadership styles and strategies to promote healthy behaviors. The role of health information technology services like tele-health and telemedicine is elaborated in Healthy People 2030. Healthcare services must provide to all individuals without social, racial, or cultural differences. The change proposal of implementing store and forward technology goals aligns with Healthy people 2030 for providing affordable and cost-effective care services to all people (Office of Disease Prevention and Health Promotion, 2020).
Vision for Healthy Community
Providing affordable, cost-effective, and equal medical care services to everyone is the vision for creating a healthy community. The change proposal aims to increase awareness and knowledge about community members and stakeholders. The technology will help in educating the patients and enhancing awareness. The inherited benefits in the vision are on-time and affordable care services. Some risks are inherent in the vision, including privacy and security concerns due to data breaches and loss of information to unauthorized personnel (Keshta & Odeh, 2020). Effective leadership styles help implement change proposals into the organizational culture. Collaborative and transformational leadership styles are effective styles that help implement technology (Van Diggle et al., 2020). The project manager acts as a transformational leader and helps create a healthy environment by motivating and encouraging the staff members (Robbins & Davidhizar, 2020). All the team members work collaboratively according to the collaborative leadership styles to achieve the desired outcomes of improved quality and health outcomes. Both styles help sustain the proposed change of store and forward technology in the community center (St. Francis Community Center, 2023).
Stakeholder Support for the Proposed Change
The stakeholders that help implement technology are project managers, groups of nurses (assistants, informaticists, and nurse managers), healthcare experts, and corporate funding agencies and grants from organizations and associations. The interest and support of the stakeholders are essential for the successful implementation of technology. The fringe benefits include financial incentives, and health insurance coverage helps increase stakeholders’ support for the change proposal (Petkovic et al., 2020). The stakeholders positively influence the proposed change:
The project manager helps in assigning responsibilities to the members.
The nurses (assistants, informaticists, and nurse managers) collectively work to improve the health benefits.
The healthcare experts, including dermatologists, skin specialists, and dentists, will guide and provide the expert clinical decision to the primary care providers.
The health condition of members of the community will improve after the implementation of technology.
Funding from corporate agencies, grants, and private and governmental regulations help arrange funds for implementing the technology.
Providing affordable and cost-effective healthcare services to all community members is the vision of the change proposal. The stakeholders face some concerns and issues. The issues include more technology acceptance, workload, and privacy and security concerns. The privacy concerns are due to the chance of a data breach and loss of patients’ confidential information to unauthorized persons and hackers (Amaraweera & Halgamuge, 2019). The issues can be resolved by using HIPAA guidelines and privacy rules (Health Information Portability and Accountability Act) (Mbonihankuye et al., 2019). The HIPAA guidelines provide information about the individual login system to avoid the chance of information loss. A study by Moore & Frye (2019) evaluated that by following the HIPAA rules, the patient’s medical information can be protected for improving health outcomes.
