With patient education being more important now than ever investing in an advanced  education technology system is of utmost importance. Patient education promotes patient  centered care and has been proven to increase medication and treatment adherence, but with out  advanced education technology it is impossible to reach these goals (Rushton et, al., 2017). With an effective patient education system, health care professionals have been able to decrease costly  readmission rates and promote efficient and cost effective care. With proper education technology patients can learn how to self-care and avoid unnecessary trips to the hospital. Patient  education programs can help the patient better understand their condition and can help lower  healthcare cost because through technology the patient and/or care giver will have a better  understanding of what to expect and what to do when things occur (Rushton et, al., 2017). This  annotated bibliography provides insight into how patient education technology can promote  quality of care and increase patient safety, patient satisfaction and outcomes.

This article discusses that with a current and up to date patient centered education system, effective patient education can be delivered as part of the discharge plan. The author’s state that having an education technology system available, individualized patient  education can help reduce diagnosis related depression and anxiety which can lead to  post discharge problems as well as readmissions. Using an accessible and customizable patient education technology system, person centered patient education can empower the 

patient to have the confidence to care for themselves and help them stay healthier at  home. This article highlights the fact that without having education technology costly  hospital readmission rates can have a negative impact on already stretched resources. By  empowering our patients to take an active role in their care through tailored patient  education technology we promote patient centered care which increases confidence and  self-care management. This article states that even with various teaching and learning  styles, having an individualized patient education technology system nurses can empower  their patient to learn and understand their disease processes and reduce preventable  complications post discharge as well as minimize depression and anxiety. Implementing an education system that focuses on patient centered education will provide the potential  to help health care professionals adequately prepare patients for a successful discharge. Individualized patient education will encompass direct or indirect care between the  provider and the patient, family or caregiver. Individualized pre and post discharge  education technology will be an effective way to help keep patients safe and healthy.  Having the technology to enable health care professionals to be able to deliver quality  education even after discharge will help keep patients safe at home.

It discusses the advantages of

increases the chances of poor adherence and is independently correlated with lack of knowledge about a medication or treatment. Studies have shown that by delivering quality education through a system that has the ability to provide multiple delivery forms can increase patient understanding thus resulting in improved adherence. By providing person centered education through video technology along with printed material and demonstration, health care professionals are able to provide the education through technology in ways that most can understand, taking into consideration various learning styles. With easy to read technology patients have a better understanding of the problem and the treatment patients are more likely to adhere to the regimen and have a better outcome with a decreased change of disease relapse or readmission to a hospital. and improve their quality of life. The authors state that online patient education technology is critical to help patients make necessary lifestyle changes and adhere to  medication and treatment. When implementing a customized patient education system it  is crucial to promote patient involvement while assessing the patients’ motivation and  skill level to ensure success post discharge. Implementing an electronic patient education  data base that will deliver easy to understand individualized patient education can increase patient satisfaction, quality of care and decrease costly readmissions. provide education that can be read at the most at a fifth grade level without using medical terms. In order to combat language and literacy levels, as well as cultural or emotional barriers it is important to be flexible with teaching material, use visual aids or verbal explanations when needed. Having online education data base technology can provide this. It is proven that using the, teach back method through technology, is an effective way to gauge understanding. Using this method the patient and/or caregiver verbalizes back or answers technology based question about any teaching given to them. Enhanced educational technology can help patients and caregivers improve outcomes while decreasing hospital readmissions. 

Conclusion 

Technology is the key to the future and having an enhanced individualized patient education  program using technology has been proven to improve patient outcomes and adherence to their  plan of care. Implementing a personalized patient education data base can empower patients to  live longer and healthier at home. Investing in an online patient education system that is easily  understood and available with the capabilities of being person centered can be costly but can  easily be compensated by, improving patient satisfaction scores, reducing hospital readmissions  and reducing the length of stay. Hence, having an individualized patient education system that  can be utilized during hospitalization and post discharge is extremely valuable to the hospital,  helping patients live safely and happily at home.
