It will ensure physicians and the facility coordinate patients’ healthcare across various settings for different providers.

Mercy medical center must also practice significant patient engagement and education.
Lack of communication between patients and practitioners increases readmissions.
Therefore, mercy medical must engage patients in their treatment during hospitalization and post-discharge care (Sharma et al., 2018)
Patient engagement and education will engage patients in the discharge process, receive correct medical instructions, understand the correct medication regimen, and have clear follow-up instructions.

 

Authorities’ Benchmarks Addressed by the Proposed Policy
In Minnesota, the Resident Public Health Act is a policy that specifies the accountability, practices, and accountability requirements for the state and local government in the province to rise patient safety and results (Minnesota Department of Health, 2018)
Mercy Medical Center, has designed organizational standards signifying readmission rates for diabetes, making the hospital be unsuccessful in realizing excellent patient outcomes.


High Priority Stakeholders
Doctors and the nurses as practitioners are are important in implementing the policies because they are the frontline workers dealing with the patients (Kang et al., 2018).

There is alternative to this perspective. The current situation of high readmission rates necessitates the implementation of the policy to effect the practice guidelines to reduce readmissions.
Staff compliance will reflect management’s success in pushing for the implementation of the changes.

In this approach, the management will collaborate with hospital staff members to implement the policy and practice guidelines and delegate duties to various hospital workers in the different departments.
However, it is possible that the management could oppose this policy and practice guidelines.
To counter this opposition, it is ideal to show the gravity of patient readmissions to the patients and the hospital to convince the management to comply (Berta et al., 2022).

In implementing the proposal, the management will source the required resources and provide them to the hospital and delegate various duties to hospital staff members towards the policy implementation.
The management will also follow up to ensure hospital staff complies with the policy.
The management’s collaboration in implementing the policy is important because it is the organization’s oversight (Bokhour et al., 2018) . It guides what happens in the hospital, and without its guidance, the hospital cannot implement the proposed policy.
