An electronic health record (EHR) monitoring and productivity-tracking system is one of the examples of an IT application employed in numerous healthcare organizations that brings ethical concerns. One of the greatest ethical concerns is that not all the information regarding the extent of monitoring is transparent. Employees have no idea that each click, delay, and documentation pattern is under discussion. Naamati-Schneider et al. (2024) note that overuse of digital surveillance in healthcare settings takes away professional autonomy and is a factor in moral distress.

Nurses who are under stress due to productivity indicators might compromise quality documentation. Rather than give a full-scale evaluation to patients or write in a manner that is significantly detailed, the focus can be based on time-based performance expectations. This risks the ethical principle of beneficence because whatever affects quality will also affect patient safety. Privacy is another concern. Although the information that is tracked is workflow-related and does not discuss personal data, the fact that they are not certain about the purpose of data tracking may cause the staff to believe that they are under constant surveillance. This is a violation of the moral principle of respecting persons. It also develops a punitive organizational culture as opposed to an enabling one.

The indirect effects they have on the stakeholders can be severe. In the case of nurses, continuous monitoring can lead to burnout, job dissatisfaction, and a lack of trust in the leadership (Siegel et al., 2022). The employees will reduce their engagement when they believe that they are being rated by algorithms instead of professional judgment. Rushing to write the chart or the fear of making mistakes may lead to incomplete assessment of the patient, communication delay, and the chance of an error in the documentation for the patient. 

There is also a negative impact on leadership: excessive dependence on online surveillance undermines working relationships and can lead to turnover, and this, in the end, will influence the quality of results. Balancing monitoring with trust-based leadership practices is therefore essential to maintaining staff morale. On the whole, the opportunities to track productivity in the context of EHR systems can be used to complement operational objectives, but they should be applied transparently and in accordance with the requirements of ethical values.
